Position Overview:

As a Home Health Physical Therapist , you will:

• Evaluate Patients: Assess patients' physical, cognitive, and functional abilities to develop individualized treatment plans.
• Develop Treatment Plans: Create and implement therapy programs aimed at improving daily living and work skills.
• Provide Therapeutic Interventions: Utilize evidence-based techniques to enhance patients' independence and quality of life.
• Educate Patients and Families: Offer guidance on adaptive equipment and home modifications to ensure a safe environment.
• Collaborate with Healthcare Team: Work closely with physicians, nurses, and other therapists to coordinate comprehensive care.
• Document Progress: Maintain accurate records of patient evaluations, treatment plans, and outcomes.

What We're Looking For:

• Licensed Professional: Active license and registration New York State.
• Experience: experience in physical therapy; home health experience is preferred.
• Communication Skills: Excellent interpersonal skills to effectively interact with patients, families, and team members.
• Reliability: Strong organizational skills and the ability to work independently in a home setting.
